Concept is simple: When you get a video, tell us. The term video was used instead of dvd as some people still collect other sources such as laserdiscs, RCA videodiscs, vcds, Betamax tapes, VHS, ect. The term acquisition was used instead of purchase as you might get some discs through trade, as screeners, for gifts, ect.
Information to include is up to you. Personally, I'd recommend using the following template:
Title: (File Name)
Format: (VHS, DVD, Ect)
Edition: (R1 Special Edition, R0 Definitive Edition, ect)
Label: (Of release company, not original studio)
Cost: (What you paid for it)
Source: (Where you got it from)
Other Notes: (Print status, print limit number, spine number, number of discs/tapes in set, bonus box set items, ect)

So, to get things started, my latest purchase:
Title: Original Star Wars Trilogy
Format: DVD-R
Edition: Definitive Edition
Label: N/A (unauthorized fan release)
Cost: $30 US
Source: Epier
Other Notes: 9 disc set. Contains all original theatrical cuts in OAR.
Ordered it last night. Still waiting for it to arrive in the mail.
